Do you mean your registration? If yes, you should go to FMS, Federal migration Sergvice office, some of them a re located at same office where Russians do their passports.

Or you can get a form at any post office.

You can register at any post office with your landlord. You need to take:
a copy of your passport info page and your landlord's
a copy of your visa
a copy of your immigration card
a copy of your landlord's ownership docs of the apartment if s/he isn't registered in the apartment
two copies of this registration form:uvedomlenie.doc
Your landlord must sign both forms.
The post office employee will tell you the address of your local FMS, where you should send, by first class/registered mail, these forms.
The post office employee will stamp the lower portion of one of the forms (the other one gets sent with all your copies to the FMS) and hands it back to you, with the receipts for paying for the first-class mail. This is your registration. A few weeks later, you should receive in the mail the confirmation of delivery to the FMS of your docs.

No, you need chief FMS office for your region (its 9 of them in Moscow), not every FMS.
Go to the local FMS and ask there the address of FMS for temp-res. permit.

Find them in internet, fill it and bring it on a flash card there. In many FMS they accept electronic forms. They will help you to fix mistakes (which will be many) and print by themselves. You will need to sign it.

Where can you find them on the internet? Also there are already quite a few threads about this, maybe we should try and keep all the info in one place to make it easier to find and to avoid repeating things...?

Good luck with the application

My appologies to all, many of us refer to OVIR as FMS. So when I for example have stated OVIR as FMS. Why did State OVIR, because certain Russians still refer to the old name when translating to someone in English. Old names don't die overnight I'm afraid.
